• The voucher is valid and redeemable from September 23, 2011 to December 23, 2011, except on legal holidays.
• It cannot be used or combined with any other discount card or promos.
• Although the voucher is transferrable and may be given as a gift, the name of the patient must appear on the voucher upon redemption.
